Start a new game, and examine the chubby woman in the pool 15 times (select the second option, then select the second option again). She will then present a debug menu with a visual test, a music test and a voice test in that order.
Once in the menu, enter each option and exit out of them in order twice (visual test -> music test -> voice test -> visual test -> music test -> voice test) to add two extra menu options that serve as mission and area selects, as shown in the second screenshot.
